If you use DocuForms for 60 days, you will enjoy:

Non-Imprinted Forms

If at the end of 60 days you can bear to part with the any unopened non- personalized packages of DocuForms you ordered, you may return them at your expense freight prepaid, for exchange, credit or refund.

Imprinted Forms

Personalized or custom versions of forms that are defective may be retuned for replacement at DocuFroms’ expense (we will supply you with prepaid shipping labels) if you notify DocuForms within 60 days of receipt. DocuForms is not responsible for incorrect information furnished by customer or that appear on forms, if customer has been supplied a proof with incorrect information that customer has approved for printing.Cancelation or changes to orders involving DocuForms creating custom personalized artwork are not permitted once DocuForms begins the process of creating the artwork. Likewise cancelations or changes are not permitted on reprints of existing artwork once the printing process begins.DocuForms is not responsible for delays in delivering orders because customer fails to respond to proof transmittals in a timely manner and or fails to provide DocuForms with good contact information for delivering and following up of proofs.DocuForms reserves the right to charge customer for copy changes resulting from customer suppling incorrect imprint information or instructions and or requesting copy changes after the initial proof is produced.

Return Requirements & Procedures 

All returns must have a Return Authorization (RMA) number on the shipping label. To obtain a RMA number and instruction on where to ship the product call our Customer Service Department at 1-800-995-2001 M-F 8AM to 5PM Pacific Time.

RMA numbers are valid for 10 days from date of issuance. We are unable to accept returns without an RMA number.

  • Shipping and insurance charges are not refunded.
  • To receive a refund, credit, or exchange your purchase must be returned with shipping and insurance pre-paid in “as new” condition, be the current version of the product shipping, in original box and packaging. Any variance from these conditions could result in a restocking fee, delay or forfeiture of your return privileges.
  • Refunds on purchases made with a credit card will only be credited to the same card.
  • Refunds or credits are issued after receipt and inspection of returned goods.
  • Refunds or credits do not include original shipping and handling charges.
  • Shipping fees will not be refunded if shipment is refused at time of delivery.
  • Please remember that any address correction, redelivery, customs clearing, duties and taxes are not included in the product price or freight calculations and are the responsibility of the customer.

Any product returned without promotional item(s) included with the original transaction (e.g., buy X get Y will have the value of the promotional item deducted from the refund amount.
